The “Registration” and “Information for 2012” documents are available:

  1. Get the documentation that you need by clicking on the “REGISTRATION” button in the margin to the left of this page.
  2. Print it using the PDF print feature. The first page is the “Registration” document and the second page is the “Information Sheet for 2012” document.

 

To register:

  1. Keep the “Information Sheet for 2012” note for rules and regulations.
  2. Fill in the Registration form and submit the entire page. Do not cut the top section off!
  3. Drop off the filled in form or mail it in as instructed in the form.
